To make an online submission:
- Create an account on the website (if you do not already have one), log in, and then return to this page.
- To prevent loss of work, use SAVE DRAFT at the bottom of each page at regular intervals. To resume an unfinished submission, make sure you are logged in to the website (see top right-hand corner), and return to this page.
- Review and print your submission on the final page before using the SUBMIT button. Once you submit, you will not be able to retrieve this submission.
